Palliative Care Registered Nurse Our Company: Illumia Health is an expanding division of Gentiva, a leading provider in hospice and palliative care. Our clinical teams deliver advanced illness management, a specialized form of high-touch palliative care designed to provide relief from the symptoms and stress of illness while improving quality of life for patients in any setting. We are seeking a compassionate and experienced Registered Nurse to join our team and provide high-quality end of life care to our patients. Key Responsibilities:

• Conduct in-person and telephonic assessments for palliative care patients
• Complete risk stratification assessments and offer education to patients and families
• Collaborate with the interdisciplinary team to coordinate care and support patient comfort
• Communicate clinical findings and document timely clinical records Requirements:
  • Registered Nurse license to practice in the state
  • 2 years of prior skilled Registered Nurse experience
  • Nursing experience in palliative care, oncology, hospice, or geriatric care preferred
  • Strong compassionate phone skills and excellent written and verbal communication We Offer:
